Interprofessional Care Project

Hamilton Urban Core is a thriving inner city health centre and a leader in the provision of high-quality primary health services and health promotion programs.  We are seeking 3 individuals to work with our exciting project which is focused on developing a framework for improving interprofessional care in a primary health care setting, enhancing team effectiveness, and identifying and building on core competencies.  This is an excellent opportunity to be part of a dynamic team and contribute to improving health outcomes for individuals and families in Hamilton.  This is a one year project.

Project Coordinator (full-time)

Working across multidisciplinary teams of health professionals the Project Coordinator will be responsible for providing leadership to the project and ensuring the intended outcomes are achieved.  

Qualifications:

  • Demonstrated working experience in project leadership, management, and implementation
  • Good knowledge and understanding of primary health care and chronic disease prevention and management preferably in a community-based setting
  • Solid understanding of multidisciplinary and interprofessional approaches
  • Experience in identifying learning needs, and engaging adult education techniques and styles
  • Demonstrated experience leading and facilitating effective team planning
  • Excellent analytical, organizational and communication skills
  • Commitment to the anti-racism and anti-oppression principles at Hamilton Urban Core CHC
  • Degree from a recognized university or a combination of relevant education and work experience

Evaluation Coordinator  (full-time)

Working with the Project Coordinator and across multidisciplinary teams of health professionals the Evaluation Coordinator will develop and implement the evaluation framework for the project and be responsible for the evaluation and analysis of performance and outcomes.

Qualifications:

  • Demonstrated experience with conventional, community-based and alternate research and project evaluation methodologies
  • Experience developing and implementing research and evaluation frameworks
  • Working knowledge of primary health care sectors; knowledge of the community health centre sector a definite asset
  • Commitment to the anti-racism and anti-oppression principles at Hamilton Urban Core CHC
  • Excellent analytical, organizational, and written and oral communication skills  
  • Excellent knowledge of computer software programs to support evaluation and research
  • Degree from a recognized university or a combination of relevant education and work experience

Administrative Assistant (full-time)

The Administrative Assistant will provide infrastructure support for the Project including administrative, clerical and secretarial support, resources management, data entry and supporting communication and correspondence for the Project.  

Qualifications:

  • Demonstrated experience maintaining a high level of efficiency through administrative practices
  • Knowledge and proficiency in the use of computers and various software programs
  • Solid organizational skills and ability to work independently
  • Excellent written and spoken communication skills, and sound problem solving abilities
  • Demonstrated experience providing direct project or team support
  • Two to four years administrative experience
  • Relevant education and work experience
  • Commitment to the anti-racism and anti-oppression principles at Hamilton Urban Core CHC
