Primary Care Practice Improvement Facilitators, Hamilton Family Health Team

The Hamilton Family Health Team (HFHT) comprises of 119 family physicians serving a patient population of approximately 250,000. We are currently looking for the following professionals to join our team.

Primary Care Practice Improvement Facilitators

You will provide leadership in the design, development, implementation and coordination of all practice improvement initiatives. Your duties will include supporting practice sites with the integration of multi-disciplinary teams and the management and delivery of chronic disease programs.  You will work closely with the practice team to maximize full scope of practice and effectiveness in achieving the strategic goals of the HFHT members.  The position is based out of the HFHT central office at 10 George St., Hamilton, ON.

A degree in a health science, administration or a relevant social science combined with clinical experience in the health or community services sector is required. Expertise acquired through training in adult education, leadership, coaching, facilitation and change management is desirable. Experience with quality improvement methodology is an important asset along with familiarity with  primary health care. Knowledge of chronic disease management and self-management support is considered an asset for these positions. Exemplary interpersonal skills, excellent communication and problem solving abilities, comfort with information technology, flexibility and ability to work independently are essential. You must have a valid driver's license and be prepared to travel and adapt to work with interdisciplinary teams in multiple locations.
