CAPTURE Needs Assessment Survey

If you are involved in health promotion planning and/or evaluation, program delivery or policy development Have Your Voice Heard about the supports needed to facilitate evaluation practice and the sharing of real world evidence.

The CAPTURE Project (Canadian platform to increase usage of real-world evidence) is building a web-based platform to support the evaluation of chronic disease prevention programs and policies. The platform will enable the collection, sharing and use of qualitative and quantitative evidence in order to improve our understanding of what interventions work, for whom and in what contexts.

The survey takes approximately15 minutes. Those completing the survey are invited to enter a draw for one of two $50 Chapters Bookstore gift certificates. Your participation and responses will remain strictly confidential. No individual data will be reported.
