PROGRAM AREA: Health Information Systems
EMPLOYMENT STATUS: Part-time
HOURS/WEEK: 17.5 - 24.5 hours/ week
You will work closely with the other members of the Health Information Systems Team and staff across Guelph CHC to analyze health data to assist in program planning, development and evaluation. The Health Planner, working with staff and community partners, will identify, develop and oversee program, service and other evaluations to ensure continuous quality improvement and accountability to the management team, community and funders. Combining your excellent interpersonal, organizational and communication skills you will participate on internal projects to build staff and agency capacity to enhance planning, evaluation and quality of health and social services. Additional responsibilities include coordinating Guelph CHC's contribution to external academic research.
You have a university degree in health or social services, with graduate training focused on health promotion, administration, and/or planning and evaluation, along with 3-5 years of relevant experience in a community based, multidisciplinary health or social services environment. Your program planning and evaluation experience, along with your strong knowledge base of quality assurance, research methodology, current health literature, best practices, and analytical skills makes you the ideal candidate. You have a clear understanding of the determinants of health, especially as they relate to our priority groups and have demonstrated the ability to work both independently and as part of a multidisciplinary team. You are proficient with personal computer systems, including Microsoft Office, and have some understanding of SPSS and database applications.
