RFP--Project Evaluation & Literature Review, Bangladeshi-Canadian Peer Outreach Project, Diabetes Education Community Network of East Toronto

The Diabetes Education Community Network of East Toronto’s mandate is to provide diabetes self-management supports and services to adults with type 2 diabetes in East Toronto.   One of DEC NET’s priority populations is a large Bangladeshi-Canadian community in the Crescent Town/Victoria Village Neighbourhood.  Following a needs assessment, DEC NET embarked on a Peer Outreach initiative – the aim of which is the creation and implementation of training manual and toolkit for conducting diabetes peer outreach in this community.  


  • Literature review of best practices in culturally appropriate peer outreach for chronic disease management and prevention;
  • Creation of evaluation framework articulating short and long-term objectives and indicators;
  • Development/selection of indicators reflecting improvement in health beliefs and self-care practices to be included in final toolkit.

Ability to speak Bangla is not required.
All work is to be completed by October 30, 2010.
Budget: $3500
