Better Living Health and Community Services (http://www.betterlivinghealth.org) has just released “From Hospital to Home,” a new episode in its “Gettin’ Up There” web series to help older adults and seniors get the information they need to make the safe transition from hospital to home and to avoid unnecessary readmission to hospital.
Using a light-hearted approach, this six-minute video follows host Katherine McLeod as she takes a tumble and has a brief stay in the hospital. By talking with a social worker responsible for discharge planning, Katherine learns all about the services and support she’ll need at home and how to get them.

According to The Change Foundation, an Ontario health policy think tank, there were almost one million discharges from Ontario hospitals in 2006. Recent research reveals that many elderly patients who are discharged lack clear and complete information to help them understand what is available to help them when they get home. They need help to navigate a complex, confusing system.
