Social Worker, Middlesex-London Health Unit

Family Health Services has an exciting opportunity for an experienced Social Worker for a full-time assignment. The Social Worker will participate in the delivery of the Healthy Babies Healthy Children (HBHC) Program working with Public Health Nurses and Family Home Visitors providing home visits to families in London and Middlesex County. 

The successful candidate will:

  • Provide service coordination, counseling and referrals to high-risk families on such issues as immigration and settlement, financial resources and planning, and employment and educational attainment, and other areas which may from time to time become evident.
  • Maintain an excellent level of knowledge of the Public Health Program Standards, HBHC protocols, and resources available for clients, adult and child learning principles, facilitation strategies, and the field of family health with a specific focus on priority populations.
  • Participate in sharing client data, problem solving challenging situations, advocating for client needs and contributing towards the development of the family service plan
  • Carry out the mandate of this role professionally, accurately and with minimal supervisory support.  Must be self-directed in moving forward health promotion strategies, able to independently negotiate workable solutions to conflicting demands, and able to set priorities for work that meet individual, program, community and health unit needs.
  • Facilitate community decision making within the context of service priority criteria

Requirements include a Bachelor of Social Work degree (B.S.W.) and registration in good standing as a Social Worker in Ontario. At least one year of pertinent work experience is required.  Candidates will have leadership skills, an understanding of the impact of social determinants of health, the ability to work in a fast paced interdisciplinary environment, an intermediate level of computer skills, and the ability to collaborate with professionals within multi-disciplinary environments. Facilitate community decision making in right of program priorities.  Excellent verbal and interpersonal skills, including strong interviewing, observation, and listening skills are important in this position. The ability to communicate in French or other languages would be an asset. A valid Ontario Driver’s Licence and use of a reliable vehicle is required as the position involves daily travel to all parts of the city and county.

Salary range for this position is $51,562 to $64,453 per annum.
