The goal of the FASD Initiative in Peel is to support the maintenance of a collaborative service model for FASD, based on best practices for Peel, utilizing the established virtual clinical service team.  The provision of quality service and support to facilitate a cultural shift will be key activities for the duration of this work.  The Initiative's efforts in addressing issues and requirements, from a  service and funding perspective, will be vital to ensure comprehensive service to clients and their families, occurs.

Position: Fetal Alcohol Spectrum Disorder (FASD) Program Coordinator- .70 FTE (24.5 hours per week)
Available: September 2011 to approximately June 26, 2015- Contract

Responsibilities:

  • Assume leadership for program development, implementation and coordination of the FASD Program for the Region of Peel
  • Support the development and maintenance of a community based, comprehensive and sustainable continuum of services, which is accessible to families experiencing FASD
  • Develop and maintain a network of contacts among service providers, sponsors, Steering Committee and key personnel on FASD-related projects and programs
  • Coordinate and support the FASD virtual Clinical Service team and stakeholders
  • Plan, organize, direct and evaluate administrative tasks related to FASD prevention, diagnosis and intervention, as well as education and research activities related to the program
  • Develop a work plan, annually, to guide the FASD program’s development and expansion, in collaboration with service providers/stakeholders
  • Facilitate the development of service protocols through collaboration with community partners
  • Maintain the Peel FASD Clinical Service database
  • Coordinate, organize and conduct presentations to community partners to provide information on FASD and available services
  • Liaise with other FASD clinics/services to exchange information and share resources
  • Support the monthly FASD Parent Support Group in Peel, as required
  • Liaise with Ministry personnel and other funders, as applicable, in the establishment of a sustainable FASD service in Peel Region
  • Work collaboratively with local planning tables, including PCYI, in the development of shared activities and planning
  • Coordinate and participate in Peel FASD Steering Committee meetings and provide statistical reports, as required
  • Coordinate Peel FASD Clinical Service meetings, as required
  • Complete all necessary reports to Ontario Trillium Foundation and Success by 6 Peel, as required
  • Work flexible hours; regular evening and weekend work is required
  • Provide own transportation and travel, as required
  • Participate in staff orientation and training, team and AYSP meetings and clinical case conferences/support on a regularly, scheduled basis
  • Is accountable to and supervised by the AYSP Clinical Manager
  • May assume from time to time other responsibilities related to the operation of the program/AYSP

Qualifications:

  • Minimum undergraduate degree in Nursing, Social Work or a related profession, or an equivalent combination of education and work experience. Master’s degree preferred
  • Demonstrated commitment to the FASD program’s objectives and knowledgeable about FASD services and client needs
  • Possess an understanding of FASD issues in the Region of Peel
  • Relevant experience combining clinical expertise and leadership qualities in an interdisciplinary environment
  • Possess the ability to develop and maintain effective working relationships with a wide variety of stakeholders
  • Experience in program development, implementation and evaluation
  • Possess sound research, analytical and report writing skills
  • Strong project management and communication skills
  • Work flexible hours, including some evening and weekend work
  • Possess a valid Ontario Driver’s License in good standing and access to a reliable vehicle
