This senior level director performs a key leadership and project management role in leading a series of inter-related hospital and community-based initiatives and projects that impact the lives of seniors and adults with complex needs through improved Access to Care.

Access to Care is a transition management philosophy and approach focused on keeping patients; specifically seniors and adults with complex needs; safe in their homes for as long as possible with community supports by aligning resources and access across the continuum of care to meet healthcare needs of patients and their families.  

Working closely with the LHIN, CCAC and system delivery partners, you will provide leadership and project management expertise for three multifaceted projects within the scope of Access to Care.  In this role, you will prepare master project plans, work with system partners in achieving project scoping requirements, coordinate and work with multiple project teams to balance multiple work plans and schedules to ensure deliverables are met, identify and mitigate risks associated with meeting project goals and develop partnerships and relationships vital to the success of the project.

The successful candidate will have a university degree in healthcare or business administration and/or equivalent education and experience.  You must also have demonstrated leadership in project management and good knowledge of performance improvement approaches used in healthcare and community care settings, as well as the ability to successfully manage relationships in a multi-stakeholder environment.    You have good knowledge of Ontario’s health care system, including the LHIN, CCAC, hospital, and community support sector priorities, policies, practices, and service standards.  Exceptional written and oral
communication and presentation skills are required.

Travel is required throughout the South West LHIN boundaries and periodically to Toronto.
