I wanted to extend a very special invitation to you to join us on Nov 14th, 2011 - World Diabetes Day, to officially launch Canada's first ever Diabetes Prevention Care Kit for South Asians!
Back in November 30 2010, Flemingdon Health Centre’s South Asian Diabetes Prevention Program (SADPP) was proud to launch the English-only prototype of this Care Kit. Fast forward to one year later, this time around, we are launching the multi-lingual versions of the kit (English & Tamil, English & Urdu)! The South Asian Diabetes Prevention Care Kit provides South Asians seniors at risk of diabetes with a practical set of tools to take control over their personal health and the health of their communities.
Diabetes disproportionately affects South Asian communities as a result of socio-economic, genetic and lifestyle factors. This program reduces inequities faced by different populations in Toronto by offering an accessible program tailored to meet the needs seniors in South Asian communities.
The kit was created with support from the Toronto Central Local Health Integration Network and funding from the Ministry of Health and Long-Term Care’s Aging at Home Strategy, which assists seniors to live independently at home and in the community.
Please share the link with your networks. We will be scheduling clinics and workshops geared to the South Asian communities from Dec 2011 through March 2012 and have a limited number of kits which we want to make available to as many Urdu-speaking and Tamil-speaking communities as possible. The kits are to be provided to participants who attend the Early-Detection Clinic and Workshop sessions offered by SADPP in the community.
