We are seeking a dynamic Primary Health Care Nurse Practitioner to join a clinical team dedicated to providing health care to individuals who are homeless or at risk of homelessness.  This position offers a unique opportunity to make a difference by facilitating change with people significantly marginalized due to poverty, addiction, mental health and housing issues. The Nurse Practitioner works as a member of an interdisciplinary primary care team of physicians and nurses.  This team, in turn, is supported by social workers, dietitians, outreach workers, community developers and support staff specializing in the areas of Mental Health Care, Children, Youth & Families, Immigrants & Ethno-cultural Communities, Poverty & Homelessness and Diabetes & Chronic Diseases.

The successful candidate also has the opportunity to initiate, participate and /or lead programs, presentations, evaluations and other system changes to continue the enhancement of interdisciplinary care for vulnerable persons.
