SADPP Registered Dietitian Position, Flemingdon Health Centre

Flemingdon Health Centre, a community health centre since 1975, is a fully accredited, non-profit charitable community health centre (CHC) located in Toronto, Ontario serving our clients within a defined geographic area. We are governed by a volunteer Board of Directors consisting primarily of residents and/or clients.
The South Asian Diabetes Prevention Program was started in 2009 with the overall goal of the providing the South Asian communities with equitable access to services by offering language-specific and culturally sensitive diabetes prevention and management services.We are seeking a Registered Dietitian to work collaboratively within a team of a Registered Nurse, Community Outreach Workers and a Program Coordinator on a comprehensive, community based Diabetes prevention program targeting the South Asian community.

Employment terms: Part time (21hours/week, with the expectation of working some evenings and weekends)

Reporting to: Program Coordinator

Expected Start date: January 11th 2012

Qualifications, Skills and Proficiencies:

  • Registration with the College of Dieticians of Ontario
  • Experience in diabetes education in a community setting is a significant asset
  • Certification in as Diabetes Educator (or working towards completing the designation) is preferred
  • Sensitivity and knowledge of the needs of multi-ethnic community
  • Demonstrated experience in adult-education
  • Well organized, ability to work well independently and as a team member
  • Familiarity with computer technology is needed
  • Ability to speak one or more of the South Asian languages is required
  • Experience working with a multidisciplinary team
  • Willingness to work in non-clinical settings and ability to travel offsite are requirements of the position
  • Must be skilled in the knowledge of disease prevention with good understanding of the role of health promotion
  • Ability to work collaboratively with other sectors in order to address the social and environmental factors that inhibit health and well-being pertaining to Diabetes


  • Plan, deliver, and evaluate diabetes education programs and community initiatives for the South Asian community in collaboration with the project team members. Programs may include: group teaching, support groups, community kitchens, and other initiatives
  • Develop, organize and facilitate community kitchen sessions to promote healthy eating
  • Participate in conducting mass screening for pre diabetes
  • Refer clients to other services for further care
  • Meet monthly performance targets of the project
  • Explore and seek collaborative opportunities with relevant organizations
  • Develop and evaluate educational resources
  • Participate in team meetings and centre-wide initiatives
  • Perform administrative tasks, mmaintain accurate records and write monthly reports
  • Participate in the development and evaluation of the project
