SADPP Registered Dietitian Position, Flemingdon Health Centre

Flemingdon Health Centre, a community health centre since 1975, is a fully accredited, non-profit charitable community health centre (CHC) located in Toronto, Ontario serving our clients within a defined geographic area. We are governed by a volunteer Board of Directors consisting primarily of residents and/or clients.

The South Asian Diabetes Prevention Program was started in 2009 with the overall goal of the providing the South Asian communities with equitable access to services by offering language-specific and culturally-relevant diabetes prevention and management services.We are seeking a Registered Dietitian to work collaboratively within a team of a Registered , outreach workers and a Program Coordinator on a comprehensive, community based Diabetes prevention program targeting the South Asian community.  SADPP is funded by the Ministry of Health and Long-Term Care’s Ontario Diabetes Strategy.

Employment terms: Part-Time (21hours/week, with the expectation of working some evenings and weekends)

Reporting to: Program Coordinator

Qualifications, Skills and Proficiencies:

  • Registration with the College of Dietitians of Ontario in good standing.
  • Experience in diabetes education in a community setting is a significant asset
  • Certification in as Diabetes Educator (CDE) or working towards completing the designation is preferred
  • Sensitivity and knowledge of the needs of multi-ethnic South Asian communities
  • Demonstrated  experience or certification in adult-education
  • Demonstrated experience or certification in  behavior change
  • Superb organizational skills and exceptionally strong team-work skills
  • Ability to speak and/or write one or more of the South Asian languages is required
  • Experience working with a multidisciplinary team
  • Ability to work collaboratively with other sectors in order to address the social and environmental factors that inhibit health and well-being pertaining to Diabetes
  • Must be skilled in the knowledge of disease prevention with good understanding of the role of health promotion
  • Willingness to work in non-clinical settings and ability to travel offsite are requirements of the position
  • Familiarity with Microsoft Office 2010, especially PowerPoint, Excel and Access

Responsibilities:

  • Plan, deliver, and evaluate diabetes education programs and community initiatives for the South Asian community in collaboration with the program team members. Programs may include: group teaching, support groups, followups, and other initiatives
  • Perform data-collection   screening tool used by the SADP program to identify those who at risk of developing diabetes
  • Develop, organize and facilitate collaborative sessions to promote healthy eating
  • Participate in conducting mass screening for pre diabetes within the South Asian communities
  • Play advocacy role for program participants in enhancing equitable access to primary care and diabetes education and management services
  • Conduct literature searches using academic electronic databases
  • In close coordination with the RN, refer and facilitate attachment to services for those at high-risk or those already living with diabetes
  • Meet monthly performance targets of the project
  • Explore and seek collaborative opportunities with relevant organizations
  • Develop and evaluate educational resources, conduct environmental scan for existing tools/resources and identify potential collaborations
  • Demonstrate innovation and creativity in leveraging the capacity to clinically outreach with other service providers aligned with scope
  • Participate in team meetings and centre-wide initiatives
  • Perform administrative tasks, maintain accurate records and write monthly reports
  • Participate in the development and evaluation of the project
