Rexdale Community Health Centre is currently seeking Full-Time Nurse Practitioner (Registered Nurse – Extended Class)

Rexdale Community Health Centre is a non-profit, multi-service centre providing primary health care, settlement and community development activities for women, families, youth, adults, and seniors of diverse cultures.  The Centre is committed to providing accessible and sustainable programs and services to diverse communities.  The Centre is currently seeking a full-time Nurse Practitioner who will promote health and prevent illness through primary care, community health services and health education.  The Centre has service delivery that incorporates principles of advanced access, utilizing group visits and self management models. We are seeking an individual with a commitment towards the above principles. The successful candidate will be a member of the Centre’s Clinical team, and be assigned to work at the main and various satellite sites of the Centre.


  • To provide health care, treatment plans, health education and support for clients
  • To participate in the development, implementation and evaluation of treatment, education, counseling and health promotion services for individuals, families and the community
  • To order/interpret results of screening, diagnostic lab tests, x-rays and ultra sounds
  • Provide clinical care to pre and post natal pregnant women and their newborns including breastfeeding support programs
  • To determine the need for and implement health promotion, primary and secondary prevention strategies for individuals, families and communities
  • Explore innovative service delivery approaches
  • Advanced knowledge and proficiency in current, evidence based methods and practices of primary care delivery with an emphasis on health promotion, risk reduction and injury prevention
  • To support and counsel clients in managing their chronic disease
  • Familiarity with client centered and self-management approaches to chronic disease prevention and management
  • Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model
  • Maintain and develop competence through appropriate continuing education methods         


  • Baccalaureate nursing degree with Extended Class Certificate of Competence from CNO in primary health care
  • Registration with the College of Nurses of Ontario with professional liability insurance
  • Affiliation with Nurse Practitioners’ Association of Ontario (NPAO)
  • Experience working as a Registered Nurse Practitioner in a community setting is an asset
  • Knowledge of Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
  • Familiarity with electronic medical record, e.g. NOD, Purkinji is an asset
  • Proficient with computer software including MS Office, Internet and email
  • Ability to work effectively both independently and as a member of an interdisciplinary team
  • Demonstrated ability to work well with women, youth, marginalized population and diverse ethno-racial and cultural groups
  • Demonstrated experience in all aspects of program development and implementation
  • Awareness of and sensitivity to the health issues of a diverse community
  • Sound knowledge of health promotion and disease prevention is an asset
  • Excellent verbal and written communication skills
  • Demonstrated ability to multi-task, and be flexible and organized
  • Must be able to provide criminal record check clearance
  • Ability and willingness to travel
  • Knowledge of the Rexdale community
  • Ability to speak a second language (e.g. Spanish, Punjabi, and Somali) will be an asset.

Salary: $74,038 to $89,203 plus excellent benefits (salary range depends on experience)   
Hours: 35 hours per week (flexible hours)
Status: non-unionized position
