Community Care Access Centres (CCACs) are publicly funded healthcare agencies dedicated to enhancing the health, quality of life and independence of individuals in our communities by offering a single point of access to home and community services. CCACs assist clients in making informed choices about health care options and work in partnership with clients and care providers including hospitals, community support agencies, LTC homes and others to ensure that the right care is available at the right time and place. As part of a system wide initiative to strengthen connections across the health continuum, the London Health Sciences Centre, in conjunction with the South West CCAC is hiring Nurse Practitioners for their Intensive Home Care Team.

Nurse Practitioner, Intensive Home Care Team

The Nurse Practitioner is a self-directed practitioner who provides holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population. The responsibilities include but are not restricted to providing palliative care for clients in their own homes as they near end of life, liaising with clients, families and all health care providers and using clinical assessment, monitoring and management skills to provide the best possible client care. The NP will be a key member of the Client Services Team, helping clients to manage pain and symptoms and avoid unnecessary hospitalization. The successful candidates will report jointly to the Program Director, Emergency Services & Base Hospital, LHSC and the Regional Client Services Manager, South West CCAC. These positions are
located across the South West region in Elgin, Oxford, London-Middlesex, Huron-Perth and Grey-Bruce.


  • Current certificate of registration with the College of Nurses of Ontario: Registered
  • Nurse Certificate of Competence with Extended Class
  • Completion of Masters of Science in Nursing (MScN) an asset
  • Completion of a Nurse Practitioner Certificate
  • Successful completion of the College of Nurses of Ontario Adult or Primary Care
  • Nurse Practitioner examinations
  • Minimum two (2) years of recent nursing experience with individuals who require end of life care
  • Familiarity with Nursing Professional PracticeModels
  • Demonstrated effective interpersonal relationship and group/team skills
  • Demonstrated excellence in written and verbal communication skills
  • Demonstrated ability to work and communicate collaboratively in an
  • interdisciplinary team environment, including ability to work with team members of diverse culture who may be providing alternative approaches to care.
  • Demonstrated leadership for the advancement of clinical practice and achievement of Program goals
  • Highly developed critical thinking skills and ability to conceptualize and analyze problems
  • Demonstrated ability to be self-directed and function independently
  • Demonstrated use of theory and research/evidence based outcomes within own practice
  • Demonstrated clinical research experience – knowledge of basic research designs, measurement techniques and statistical methods
  • Knowledge and experience in change theory and adult learning principles
  • Knowledge and experience with the long term care system
  • Demonstrated knowledge of and commitment to patient and staff safety at LHSC
  • Demonstrated ability to attend work on a regular basis
