The Communicable Diseases Investigations Team requires a self-directed individual to work within a collaborative team environment to deliver services in accordance with the Ontario Public Health Standards for Communicable Disease Control. Key responsibilities include investigating reports of communicable diseases and case management; disseminating knowledge of infection control practices and outbreak control measures; developing and maintaining linkages with other health professionals and agencies (e.g. physicians, local hospitals and community health agencies) and assisting the supervisor in program development.
Examples of job activities may include completing case management and contact follow up on reports of communicable diseases; providing direction on outbreak control measures and follow-up and developing support materials for educational purposes.
