The Haldimand-Norfolk Health Unit has undertaken a comprehensive review of all of its programs and services. The goal of the review was to ensure the provision of high impact programs and services, based on community needs and available resources to meet the Ontario Public Health Standards.
Five priority directions were identified by the service review process. To address the priority directions we reorganized our internal structure to align with the population targets. Teams were restructured to include a mix of public health professionals to bring a wider set of skills to the programs and to encourage inter-team collaboration.
The Health Unit is seeking a consultant to develop an evaluation plan to be implemented throughout 2014. The plan should include the identification of both process and outcome indicators and include templates for both interim and final reports.
a) Work collaboratively with the Health Unit Management Team to plan an evaluation of the implementation of our priority directions and our new organizational structure.
b) Develop any tools required (e.g., surveys, spreadsheets, checklists, report templates)
c) Provide a summary document outlining the step by step evaluation plan
