Program Lead
Current Focus: Rehabilitative Care and related initiatives

The South West LHIN is seeking a proven Program Lead who will provide strong leadership, project planning and coordination skills to enhance integrated local health service delivery while supporting various priorities and projects within the South West area.

As part of a broader strategy to transform our healthcare system, improve patient experience and health system outcomes, and enhance population health, you will work in collaboration with health service providers, and other LHIN partners to support strategic planning, health system design and integration, and improvement initiatives with a focus on Rehabilitative Care and related initiatives.

Reporting to the Senior Director, System Design and Integration you will identify and support planning, coordination and integration activities related to Rehabilitative Care and implementation of the provincial Assess and Restore Guideline; identify and progress performance measurements, quality improvements and integration opportunities with the LHIN stakeholders; identify and foster LHIN-wide relationships among health care and related sectors for health improvement; coordinate and provide project management support to groups responsible for overseeing the development and implementation of initiatives; and work in partnership with LHIN teams to ensure integration of project strategy with other LHIN initiatives to ensure a systems approach to health system improvement.

Qualifications: The successful candidate will have:  Demonstrated experience leading complex health care system design, quality improvement and integration initiatives:  Extensive and successful track record working with a broad range of stakeholders in the health sector combined with an in-depth understanding of Ontario’s health care system, its stakeholders and programs with a particular focus in Rehabilitative Care and related initiatives: Exceptional project management, written/oral communication and presentation skills are required; Post-secondary education in health administration, business administration and/or a related health discipline or the equivalent in education, training and experience; Proficiency in French would be an asset.
