Triple Aim is a framework developed by the Institute for Healthcare Improvement (IHI) that describes an approach to optimizing health system performance, with emphasis on three dimensions: improving the patient experience of care (including quality and satisfaction); improving the health of populations; and reducing the per capita cost of health care.

Hear from Gateway CHC which acts as lead agency for a dynamic Triple Aim collaborative designed to guide system planning and monitor/evaluate performance within the Rural Hastings Health Link, located in rural south central Ontario. Hear from US NACHC’s Chief Medical Officer who will describe insights and lessons from Triple Aim efforts across America’s network of over 9000 Community Health Center sites, 61% of which are certified Patient Centred Medical Homes (PCMH).
