Position Summary:

In support of the increasing need to focus on chronic disease prevention and management, a Health Promotion & Disease Prevention Specialist will provide leadership and work collaboratively with all members of the Family Health Team in the development and implementation of health promotion and disease prevention programs.
 
Job Responsibilities:

  • Facilitate the development of a broad range of health promotion and chronic disease management processes, programs and patient education (group and individual) in collaboration with other multidisciplinary team members
  • Develop evaluation processes to ensure outcomes of programs are being achieved
  • Build a culture within the FHT that facilitates incorporation of health promotion activities into all patient encounters
  • Create health promotion educational information
  • Strong research and planning skills
  • Strong group facilitation skills
  • Participate in teaching activities for Residents

Key Qualifications:

  • Degree in health services discipline
  • Completion of a relevant Masters graduate level program an asset
  • Demonstrated experience in health promotion and chronic disease prevention & management
  • Ability to manage multiple priorities simultaneously
  • Knowledge of and working experience with computer systems and applications
  • Excellent verbal and written communication skills
  • Ability to  work collaboratively  as a member of the multi-disciplinary team
  • Teaching Experience in a Primary health setting
  • Knowledge of the complexities of the academic environment
