WellFort (formerly Bramalea Community Health Centre) was founded eight years ago by a group of dedicated community leaders who had a vision of a health centre that would provide inclusive, accessible and innovative health programs and services to its diversely rich community. In a few short years, WellFort has emerged as a dynamic part of the Brampton community providing high-quality health services and fostering deep connections with individuals, groups and communities in Bramalea and Malton. The WellFort family of programs includes the Bloom Clinic, Bramalea CHC, the Diabetes Education Program, Four Corners Health Centre, and Health n’ Smiles.

WellFort’s work is grounded in the knowledge that there is more to health than treating people when they are sick. The agency follows the World Health Organization's holistic definition of health:  

"Health is the extent to which an individual or group is able, on one hand, to realize aspirations and satisfy needs; and, on the other hand, to change or cope with the environment.  Health is therefore seen as a resource for every day life, not the objective of living; it is a positive concept emphasizing social and personal resources, as well as physical capacity."

To achieve this complete concept of good health, WellFort has put together a team with many different skills: nurse practitioners, diabetes nurse educators, physicians, medical office assistants, dietitians, social workers, dentists, community health educators and health promoters.

Primary care focuses on health promotion and illness prevention. Community development programs respond to, and reduce, the root causes of illness and injury. And advocacy is targeted toward improving health and breaking down the barriers that prevent people from accessing the care and supports they need.

WellFort’s mission and vision guide the strategic priorities for a safe and healthy community where everyone belongs and diversity is celebrated. The way WellFort pursues that vision is unique to the CHC. Its approach to service is called Open Focused Care that is characterized by efforts to:

  • be fluid and mobile, reaching out to people who face barriers to access;
  • actively challenge and remove oppression perpetuated by power inequalities;
  • discover and mobilize every available asset to build a healthier Bramalea and Malton; and
  • create a service system that is open and accessible to clients.

Through the model of Open Focused Care, WellFort brings immediate, holistic health care to its priority populations that include youth, isolated seniors, newcomers, families who are under-resourced, members of racialized communities, people dealing with mental health and/or substance use issues and people who are homeless, under-housed or at risk of homelessness

The Role

Reporting to the Chief Executive Officer, the Manager of Human Resources is responsible for the successful leadership and management of human resources at WellFort. S/He participates as part of the leadership team to support the organization’s Vision and Mission.

The Manager’s key responsibilities are:

  • S/He provides human resource support to the organization including recruitment, staffing, training and staff development. This position provides advice and assistance to the leadership team and staff, including information on training needs and opportunities, job descriptions, performance reviews and personnel policies.

The Candidate

As the Manager of Human Resources, you will bring to WellFort your commitment to excellence, management skills, and experience working as part of a dedicated team of professionals within a community-based health or human service organization.

You will collaborate with other members of the leadership team to support and build the team’s capacity. You will establish and foster positive relationships with funders, suppliers, landlords and other stakeholders with whom the position relates.

Your commitment to clear, accurate and timely reporting; your knowledge of applicable legislation; your attention to detail; and your ability to think comprehensively, build respect and trust and provide strong corporate support for the entire organization in the area of human resources, will assist you in meeting the demands facing the organization.

As a manager, you will inspire and motivate others to succeed and help to build competent and empowered staff teams. You will demonstrate a commitment to diversity and create a culture of learning and personal growth. You are compassionate and sensitive to the issues facing WellFort’s priority populations and will work within a social justice framework to assist the Chief Executive Officer to achieve the WellFort Mission and Vision.

With a minimum of three years of progressive management experience, you are a confident leader and decision maker, committed to enhancing community health.

You have an undergraduate degree from a relevant discipline and a background in human resources management. You are proficient in the use of applicable software applications (e.g. WellFort uses Great Plains and ADP-Workforce Now).


The maximum salary is $70,000 plus a progressive benefits package including the HOOPP pension plan.
