Leader Certification Training Opportunity, Central West Self-Management Program

Peer Leader training for the Stanford Chronic Disease Self-Management Program  is being offered by the Central West Self-Management Program, supported by the Ministry of Health and Long-Term Care, as part of the Ontario Diabetes Strategy.

Participation in the 4-day Peer Leader Training will qualify you to lead Self-Management Workshops for small groups in the following communities; Brampton, Rexdale, Malton, Dufferin County, Caledon, and Woodbridge.
A Stanford Self-Management workshop series is facilitated by two Peer Leaders and is designed for small groups of adults living with, or at risk of, chronic conditions. The program is delivered through 2.5 hour workshop sessions that are scheduled once per week, over 6 consecutive weeks. As a trained Leader you commit to deliver two workshops each year.

Certified Leaders will receive a $180 honorarium for each 6 week program facilitated, once they have completed this four day Peer Leader Training.
We accept a wide range of applications—from individuals with healthcare education and experience to those individuals with little or no experience. We are seeking English, French, Punjabi, and Hindi speaking Peer Leaders.
