A Unique Opportunity to Advance Integrated Primary Health Care in Niagara
One Year Contract
Quest Community Health Centre is a not-for-profit, interdisciplinary, health care organization. We are one of 73 government funded Community Health Centres in Ontario. Our holistic model of care focuses on primary care, illness prevention, health promotion, community capacity building, and service integration, while taking into account underlying conditions such as poverty, housing problems, and unemployment.
With your skills, positive outlook and sensitivity to the needs of the community, you will facilitate development of health care services for members of Niagara’s migrant agricultural workers community.
The Community Health Worker works within an interdisciplinary team to provide community liaison and health education and promotion in addition to the developing, implementing, and evaluating initiatives that support the primary health care services to Niagara’s migrant agricultural workforce.
You have completed a college or university program in health promotion or other health-related discipline and have experience in a community setting working with interdisciplinary teams and coordinating activities or events. The successful candidate demonstrates confidence and is a skilled networker and public speaker. Experience in working with the migrant agricultural worker community and speaking Spanish is an asset.
