Quest Community Health Centre is a not-for-profit, interdisciplinary, health care organization. We are one of 73 government funded Community Health Centres in Ontario. Our holistic model of care focuses on primary care, illness prevention, health promotion, community capacity building, and services integration, while taking into account underlying conditions such as poverty, housing problems, unemployment and poor diet.
With your skills, positive outlook and clinical expertise, you are a catalyst who will help support individuals who do not have a primary care provider and who commonly experience barriers to accessing healthcare. This position works part time with Quest CHC general services as well as part time supporting the delivery of health care services for members of Niagara’s migrant worker community as an Outreach RN.
You will work independently, as well as in consultation and collaboration with physicians, nurse practitioners and other allied healthcare professionals. This unique position provides the opportunity to work in a new and evolving program area as well as continuous learning and career growth.
Your qualifications include 1-3 years experience preferably in a community health setting working with inter-disciplinary teams, or combined with hospital and or public health environment. Experience in program development, implementation, monitoring and evaluation is an asset. You are currently licensed and in good standing with the College of Nurses of Ontario. A valid Ontario driver’s license, access to reliable transportation and a Police Reference check are required.
