Building and sustaining collective health action: framework for community health practitioners

This paper describes a framework for interdisciplinary practice which is intended to provide guidance for program planning and implementation. The framework endorses the vision of health promotion described in the Charter. It translates the Charter into an action framework for Public Health programs and practitioners. The framework identifies building individual and collective capacities and action towards health, and promoting environments that support health as foci for public health interventions. Relationships among these components are highlighted and examples of the framework's application are provided.
