Community Health Worker- Health Resource & Wellness Centre


The Toronto Hospital is seeking a Community Health Worker for its community-based Health Resource & Wellness Centre. As an integral member of the Health Resource & Wellness Centre, the COMMUNITY HEALTH WORKER will develop and implement health promotion, wellness education, and community outreach initiatives; participate in community and hospital networks that address community health issues; develop and/or participate in skills and capacity building activities; advocate on behalf of clients for access to services, opportunities, resources.



The successful applicant will have: Bachelors Degree in health promotion, health education, adult education, or recognised equivalent; three (3) years of practical experience in health promotion, health education, adult education, or community development; knowledge and experience of principles of program development, implementation & evaluation; knowledge and experience of community outreach and animation; experience working with diverse communities; Excellent verbal and written communication skills; fluency in a language other than English (preferably Portuguese, Cantonese, Mandarin, Spanish, or Vietnamese); ability to work collaboratively with other program staff and community agencies; knowledge of computer systems and the internet.
