Public Health Nurse, Chronic Disease & Injury Prevention Program




Under the direction of the Director, Healthy Lifestyles, the Public Health Nurse/Heart Health Coordinator will participate in the planning,

implementing and evaluating of the activities and services of the Chronic Disease and Injury Prevention Program assigned. The position

involves extensive community collaboration and mobilization, participation in program planning, development, coordination, implementation and evaluation and consultation to the community.



A current Certificate of Registration with the College of Nurses of Ontario. A Baccalaureate degree in Nursing which includes Public Health preparation. Knowledge of/experience in group facilitation. Excellent organizational skills. Excellent communication skills (oral, written & media). Self directed, demonstrated leadership skills and excellent inter-personal skills. Valid Ontario Driver's licence and vehicle. Basic computer skills (WordPerfect 6.0) and use of Internet.



Flexible working hours are required.
