Request For Proposals Multicultural Heart Health Community Facilitator's Tool Kit

The Multicultural Heart Health Partnership (MHHP) is a growing network of partners that fosters heart health promotion within the multiculturally diverse populations of Toronto South Region. The MHHP is a program of, and receives funding from, the Toronto (South Region) Heart Health Partnership.

In the past two years, MHHP has undertaken and coordinated two major projects:

1. An Inventory of Multicultural Heart Health Educational Resources used in Ontario

2. Identifying Best Practices for Promoting Heart Health in Multicultural Communities

The Toronto (South Region) Heart Health Partnership is presently seeking professional services to develop a facilitator's tool kit to be used by community workers for planning and implementing heart health promotion programs in seven multicultural communities. The tool kit will be in English with sections culturally and linguistically adapted. The project does not entail translation of materials. The scope of the project is:

* To produce a practical, hands-on, user-friendly tool kit using recommendations from the "Identifying Best Practices for Promoting Heart Health in Multicultural Communities" report

* To link with members of the MHHP and other community partners who can provide information, ideas and resources for the development of the tool kit

* To work in collaboration with members of the MHHP to identify and investigate existing tool kits that could be modified or modeled

* To identify gaps in existing multicultural heart health educational resources in order to guide future resource development, using the "Inventory of Multicultural Heart Health Educational Resources used in Ontario" report

* To produce a written report that includes an overview of the project, key sources of consultation, resources reviewed, and recommendations for future cultural adaptations of resources and development of new resources

Other expectations include:

* Attend a minimum of 3 MHHP meetings (2 hours each)

* Formulate a detailed workplan including timelines

* Document and report on project progress at MHHP meetings as stated in the timelines

* Communicate with MHHP members as needed in order to complete the project

TIMELINES: January 28, 2002 - March 25, 2002

* Request for proposal due January 14

* First project meeting with MHHP: January 28- project workplan

* Second project meeting with MHHP: February 18- review of existing tool kits and resources with recommendations

* Third project meeting with MHHP: March 4 - presentation of 2 options for tool kit format

* Fourth project meeting with MHHP: March 25 - presentation of tool kit and final written report

DELIVERABLES: Draft versions of tool kit formats;

Final version of tool kit including resources;

Final written report as specified above

BUDGET/FUNDING: A limit of $7,500.00 has been allocated for this work including GST and PST.


* Demonstrated knowledge in health promotion, adult education, and community development

* Demonstrated ability to develop creative learning materials

* Sensitivity in working with diverse ethno-cultural communities

* Success in working with community partnerships

* Excellent verbal and written communication skills

* Self-directed, results-oriented, and able to meet established deadlines

* Experience with project management

* Competency in computer-based applications

* A current CV/statement of qualifications

* Letter outlining relevant work experience including working with multicultural communities, developing learning materials, and experience with health promotion, adult education and/or community development

* List of 2-3 references

* An application conveying the approach to be taken as well as a proposed workplan with timelines

* If selected for an interview, one example of a deliverable similar to the project (if available) is requested
