Public Health Nurse, City of Toronto, Public Health Division

Major Responsibilities

  • Reporting to the Program Manager, (Family Health or Healthy Lifestyles), you will carry out public health nursing services to prevent disease and promote health through work with groups, individuals and families
  • Assess health needs of the community and develops programs to meet those needs
  • Carry out independent home visits
  • Interview, counsels and provides health teaching
  • Assess family health status and assists the family to develop and achieve health improvement goals
  • Maintain family health histories and statistics
  • Plan and carry out health programs and special projects in the community, including schools
  • Carry out special projects and necessary services in various locations

Key Qualifications

  1. Licensed as a Registered Nurse in the Province of Ontario
  2. Baccalaureate Degree in Nursing from a recognized university with preparation in public health or a Diploma in Public Health Nursing
  3. Strong assessment skills
  4. Ability to promote health of individuals, families, groups and communities
  5. Excellent interpersonal skills and the ability to establish and maintain good personal and public relations
  6. Demonstrated ability to work in multidisciplinary teams and groups
  7. Demonstrated ability in health promotion and educational strategies
  8. Strong organizational skills
  9. Demonstrated ability to prepare reports
  10. Valid class “G” Driver’s Licence and access to a vehicle is required
