Early Years Program Coordinator, West End Toronto Community Health Centres


Three West End Toronto Community Health Centres (CHCs) are searching for a dedicated and experienced individual to implement our Early Years II (EY II) Program. The Program Coordinator will provide leadership, support, and direction in the implementation of the EYII Program and coordinate the activities of a multidisciplinary mobile team in order to achieve the integration of community health programs and services for families and children 2-6 years. The mobile staff team will include RN(EC), social workers, dietitian and community health workers. It will work with primary care and community health teams at the sponsoring CHCs to enhance existing programs and develop new initiatives.



The coordinator will ensure that the goals and objectives of the program are met; hire, coordinate, supervise and evaluate staff; facilitate connections with partner and other agencies to support program clients; provide support to the management committee and consultative structures; promote awareness of the initiative; and assist with the evaluation of program activities.



The EY II Program is funded for four years by the Ministry of Health. Partner organizations include Davenport-Perth Neighbourhood Centre, The Four Villages Community Health Centre, and Parkdale Community Health Centre.



The successful candidate will have post-graduate training in a relevant field or undergraduate degree combined with field experience, and extensive program management experience within a community-based organization; demonstrated commitment to, and knowledge of community-based health care, health promotion, and community development; extensive experience and knowledge of community consultation, program planning, delivery, and methods/models of evaluation; experience leading an inter-disciplinary team and facilitating effective team planning; ability to work flexible hours; excellent oral and written communication skills; solid computer skills; ability to speak languages other than English an asset.



This is a full-time, contract position until March 31, 2003 with expectation of renewal until at least March 31, 2006. The maximum salary is $51,383 plus excellent benefits.
