Regional Coordinator, West GTA Stroke Network

The Regional Coordinator of the West GTA Stroke Network will lead the processes involved in the commitment and system development of stroke care across the continuum of care in the west Toronto, Halton, and Peel Regions. The Regional Coordinator has dual accountability regionally to the Chair of the Regional Stroke Steering committee and in addition to the Director of the Regional Stroke Program. Provincial accountability will be to the MOHLTC Ontario Stroke strategy.

Key Qualifications

* Minimum completion of a Baccalaureate Degree in a health relation discipline; MHSc Degree or MBA Degree is preferred

* Clinical background related to stroke care and an understanding of existing processes/mechanisims in stroke care across the continuum is preferred

* Minimum five years of experience in the health care system with significant management and project experience

* Demonstrated ability to establish effective interpersonal relationships including effective collaboration with interdisciplinary teams and external regional partners

* Knowledge of data collection and information systems with strong PC and information technology skills

* Ability to promote community awareness within the community for the catchment area of the network

* Knowledge of proposal writing

* Superior analytical, problem-solving, negotiation and conflict-resolution skills

* Client-focused with a commitment to excellence


* Responsible for the overall administration of the regional network and its staff

* Develop a work plan for implementation of the regional stroke strategy

* Development of the comprehensive dissemination plan to all participating organizations

* Oversee and manage the development of the strategy in consultation with the leaders from the participating organizations

* Monitor progress and develop work plans as required throughout the region

* Participate in and facilitate the evaluation component of Network activities in consultation with the Steering Committee

* Ensure implementation of education initiatives in collaboration with the regional educator and stakeholders

* Provide support to the established working groups of the network

* Provide feedback to the appropriate MOHLTC committees as required

* Actively participate externally on committees with other Stroke Networks in Ontario addressing collaboration and coordination of stroke services across stroke regions in the province

* Monitor patient care processes established within the network to ensure their effective functioning

* Monitor quality utilization, financial and outcome indicators and monitor implementation of data to achieve established targets

* Effectively use data in future decision making

* Maintain linkages and coordination of services with community providers within and outside of the network

* Develop and sustain formal partnerships as required to meet the objectives of the network
