The South Riverdale Community Health Centre requires a Coordinator for the Diabetes Education Community Network of East Toronto (DEC NET). SRCHC is a not-for-profit organization working within a multidisciplinary primary care health team environment, governed by a community Board of Directors.
The DEC NET Program Coordinator is responsible for coordinating and supervising the development, implementation and evaluation of the program, including all management, supervisory and administrative functions in collaboration with all partners and their program staff. This position will also be involved in broader organizational initiatives in the Centre as appropriate.
The successful candidate will have a minimum of 3 years management and staff supervision experience within a community setting including working with collaborations and networks, excellent leadership, planning, communication, and supervision skills; and knowledge / experience of Primary Care issues and challenges. The applicant will need to demonstrate solid interpersonal, organizational, critical thinking competencies; and value diversity, equity, partnerships and advocacy in keeping with South Riverdale Community Health Centre's Strategic Directions. Experience and understanding of diabetes will be an asset.
