Physiotherapist, Four Villages Community Health Centre

Maternity Leave Contract - 12 months

The Four Villages Community Health Centre provides high quality, accessible primary health care and community programs. It is located in the Bloor-Keele area of west Toronto, and funded by the Ontario Ministry of Health and Long-Term Care.  Four Villages' mandate is to promote increased access to health and wellness for the communities within the service area in the context of the broader determinants of health.  A wide range of programs as well as our clinical, general health and social services, are primarily aimed at seniors, families with young children, youth and newcomers. Staff collaborate in an interdisciplinary team environment; the focus is on the client and capacity building.  We are currently seeking an experienced physiotherapist to join our dynamic team.


  • Provide assessment, diagnosis, treatment, health education and counseling for acute and chronic conditions
  • Refer clients to appropriate internal and/or external resources
  • Maintains complete and accurate client records, assessment notes and required correspondence
  • Participates in chart reviews and case conferences, and makes appropriate internal and external referrals
  • Facilitates health education in group settings to meet client and community needs


  • Master of Science in Physiotherapy from a recognized university, or an equivalent combination of relevant education and a minimum of five years experience
  • Registered under the College of Physiotherapists of Ontario
  • Eligibility for membership with the Canadian Physiotherapy Association
  • Thorough knowledge of and proficiency in current physiotherapy techniques
  • Three to five years experience in a health care setting with preferred experience in the community
  • Demonstrated ability to work effectively in an interdisciplinary team environment
  • Demonstrated ability, commitment to and knowledge of community health care;
  • Experience in the use of computers and various software applications
  • Experience in program development, implementation, monitoring and evaluation
  • Excellent interpersonal skills with clients in a culturally diverse practice population, and with colleagues
  • Excellent communication, decision-making, problem-solving, conflict management and time management skills
  • Second language, relevant to catchment area populations, is an asset

Hours of work: 17.5 hours per week
Salary: Prorated salary range of $52,846-63,670 plus payment in lieu of benefits
