Outreach Facilitator, Élisabeth Bruyère Research Institute, C.T. Lamont Centre

The Élisabeth Bruyère Research Institute, C.T. Lamont Centre is seeking an Outreach Facilitator to work in various family medicine practices across the Champlain region as part of the "Improved Delivery of Cardiovascular Disease through Outreach Facilitation" project.  This Outreach Facilitator will work on a full-time or part-time flexible schedule from October 1st, 2007 to September 31st, 2008.  Further opportunities for work may arise following this period.  The incumbents will set their hours independently based on the preference of their assigned clinics. 

The incumbent will work with the project team and assigned practice staff to assist primary care physicians with changing their structure at the system level and organizing their work toward more effective care delivery of cardiovascular disease management:

Roles and responsibilities will include

Outreach Facilitation

  • Developing a needs assessment for the individual practice site based on the site's requirements and preventive care performance
  • Developing targets with the practice site based on a needs assessment/audit for the patient population
  • Facilitating the dissemination of proven evidence based measures in the physician(s) and staff(s) daily practice
  • Facilitating the incorporation of systems proven to assist in the dissemination and implementation of preventive and cardiovascular disease management
  • Facilitating the incorporation of Continuous Quality Improvement strategies by creating audit mechanisms to review how each individual practice site is performing relative to their own requirements
  • Facilitating the incorporation of interactive continuing medical education (CME)
  • Facilitating the maintenance of accurate patient demographic and clinical data
  • Reporting to the Program Manager
  • Communicating with all relevant professional groups and organizations for the purposes of collecting information that will benefit the goals of the program

The successful candidate will have the following skills, knowledge and experience:

  • Clinical background and a Masters Degree in Health Administration/ Business Administration/ Social Sciences or other related discipline
  • Ability to travel within the Champlain region, valid driver's license and own vehicle
  • Excellent small group facilitation, organizational and interpersonal skills
  • Knowledge of basic Continuous Quality Improvement theories
  • Knowledge of current health care issues and the primary health care system
  • Ability to work in a team environment with other facilitators, project team, various physicians, nurses, allied health professionals and administrative staff
  • Ability to prioritize, coordinate several projects at one time and meet strict timelines
  • Advanced computer skills, knowledge of medical terminology and ability to learn various medical software systems
  • Knowledge of and familiarity with the family practice environment
  • Excellent judgment and decision-making
  • Outgoing personality, advanced oral and written communication skills and flexibility
  • Bilingualism (French and English) is an asset
