Feedback Required for Development of Diabetes/Disability Healthcare Professionals Resource

The Active Living Alliance for Canadians with a Disability (ALACD) is collaborating with national partners to produce guidelines and resources to address the Prevention, Detection and Management of Diabetes for Canadians with Disabilities.

We are seeking your input to prepare useful resources for practitioners that provide services for Canadians with disabilities, including those working in public health and a range of other disciplines and settings. The following survey is designed to identify the nature of your agency, community setting, your clients with disabilities, the impact of diabetes on your service provision, the type of resources you would find beneficial and the format in which the information is best made available for you.
