Practice Facilitators for Family Health Teams in Ontario, Quality Management Collaborative

The Quality Management Collaborative (QMC) is seeking facilitators to work with Family Health Teams (FHTs) across Ontario. Practice facilitators will primarily support quality improvement initiatives including Learning Collaboratives and will also assist with team building and other practice support activities. Successful candidates will be regionally based and have a contractual employment arrangement with the Quality Management Collaborative (QMC). The QMC will provide training and support to facilitators as they implement their role with FHTs. These positions are for a two year duration commencing May 1, 2007 with potential for renewal. Applicants must be able to work from a home-based office, have a valid driver's license, be prepared to travel and adapt to work with interdisciplinary teams in multiple locations.

A degree in a health science, administration or a relevant social science combined with clinical experience in the health or community services sector is required. Expertise acquired through training in adult education, leadership, coaching/mentoring and change management is desirable. Experience with quality improvement methodology is an asset along with familiarity with the Ontario health care system in particular, primary health care. Knowledge in one or more content areas such as chronic disease management, health promotion, or self-management support is considered an asset for these positions. Exemplary interpersonal skills, excellent communication and problem solving abilities, comfort with information technology, flexibility and ability to work independently are essential.
