Opportunity to Provide Feedback about the Canadian Best Practices Portal

The Public Health Agency of Canada is in the process of gathering feedback about the Canadian Best Practices Portal for Health Promotion and Chronic Disease Prevention (http://cbpp-pcpe.phac-aspc.gc.ca) and services and supports needed to better incorporate evidence into program and policy decisions related to health promotion and chronic disease prevention. A Web-based survey is underway to collect your feedback.

The survey closes Friday, May 16th at midnight EST.

To acknowledge your support for our survey, your participation qualifies you for free registration and allowable travel expenses (up to $3000 maximum total) for one person to the 2008 Chronic Disease Prevention Alliance of Canada (CDPAC) Conference in Ottawa November 24-26, 2008. You can participate in this draw by providing your email address at the end of the survey. After the winner is selected, your email address will not be used for any other purpose and it will be deleted from the survey data.
