Position:    Registered Nurse- Diabetes Educator

Status:    Full-Time

The Etobicoke Medical Centre Family Health Team is a new Primary Care Health Team, funded by the Ontario Ministry of Health and Long-term Care and built upon a longstanding medical practice. The physicians and staff of the EMC have been providing high quality care, including prevention, treatment and diagnostics, to the community in the west end of the city of Toronto and Mississauga for 40 years.  The EMC FHT will be enhancing these services by moving to a comprehensive interdisciplinary team with the addition of several allied health professionals and administrative staff.

Position Summary:
•    Contributes to the development of the clinical and program plans of the Family Health Team
•    Responsible and accountable for decisions and actions in the provision and management of nursing care for patients
•    Case management of clients with diabetes
•    Using a variety of teaching methods and principles of education, assists the client to gain the knowledge, skills, attitude and self awareness necessary to effectively manage his/her diabetes
•    Follow medical directives as per scope of practice, i..e, insulin dose adjusting, OHA adjusting
•    Maintaining ongoing patient documentation as well as government and other organizational reporting requirements
•    Participating actively as a member of the integrated Family Health Team, including training and supporting new staff, participating in problem-solving to enhance patient care.

Qualifications :
•    Certified Diabetes Educator an asset
•    BScN preferred
•    Minimum 2 years experience in counselling clients with diabetes on an individual and group basis
•    Demonstrates skills in adult education, group dynamics and counselling
•    Membership in the Diabetes Educator Section of the Canadian Diabetes Association preferred
•    Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
•    Proficiency in computer and keyboarding skills required
•    Reliable, flexible, creative professional, able to work effectively and collaboratively in a interdisciplinary team approach required
•    Experience in program development and evaluation
•    Member in good standing with College of Nurses
