Community Dietitian, Diabetes Prevention Project for Seniors, New Heights Community Health Centres

3-month Contract Position (with possibility of extension)
November 1, 2010 to January 31, 2011

Unison Health and Community Services is formed by the voluntary merger of New Heights Community Health Centres and York Community Services. Unison’s mission is: working together to deliver accessible and high quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

We are seeking a full time Registered Dietitian with experience in outreach, diabetes education and group facilitation to join a team of outreach workers and volunteers to expand the well-established, evidence-based diabetes prevention project.   

Responsibilities include the following:

  • contribute to the implementation and evaluation of the diabetes prevention project
  • conduct diabetes risk screening in the community such as church and community groups on weeknights and weekends
  • provide nutrition consultation to individuals at the screening clinics
  • deliver diabetes prevention workshops
  • conduct cooking programs
  • coordinate volunteer schedule and provide ongoing recruitment and training to volunteers
  • conduct outreach in the community
  • develop educational resources as needed
  • participate in team meetings and centre-wide initiatives
  • perform administrative tasks such as complete screening tally on a weekly basis

The successful candidate will be or demonstrate:

  • a member of the College of Dietitians of Ontario
  • strong interpersonal skills
  • excellent communication and group facilitation skills
  • excellent organizational and time management skills
  • experience working in various community settings
  • good understanding of health promotion and social determinants of health
  • experience in mentoring and training volunteers and students
  • experience in diabetes education is an asset
  • ability to speak a second language, such as Spanish, is a definite asset
  • ability to work some evenings and weekends, and to work out of multiple locations as required
