London InterCommunity Health Centre seeks an experienced manager for a key role within our organization. The Primary Care Manager provides leadership to three interdisciplinary primary health care clinics, mental health care, chronic disease care (Diabetes and Hepatitis C specifically) and systems navigation functions.
This is an exciting opportunity for an individual eager to implement new strategies and solutions that address the needs of highly complex and socially vulnerable communities within the context of today's evolving health care landscape.
Over the next three years we will be redefining our service delivery model, implementing new technologies, evolving our resource model to accommodate more flexible and innovative staffing and developing formal partnerships with academic institutions. There is an important role for the Primary Care Manager in each of these growth areas.
If you are experienced in quality improvement, have a demonstrated capacity to nurture innovation, and a proven track record inspiring interdisciplinary teams, this role is for you.
The successful candidate will have a graduate level degree in a relevant discipline. A second language is considered an asset and we encourage applications from applicants reflecting the diversity of the local community.
