RFP--Ontario Needs Assessment for Chinese Ontarians Living with Heart Disease or Stroke, Heart and Stroke Foundation

The Heart and Stroke Foundation’s visionary mission is “Healthy lives free of heart disease and stroke. Together we will make it happen.” The Foundation is currently developing a national strategic plan to establish key objectives and corresponding areas of focus under which future strategies and initiatives will align.

Through a parallel process and thanks to a designated contribution from the Chinese Canadian Council of HSF, this needs assessment study for Chinese Ontarians living with cardio- and cerebro-vascular diseases (CCVD) will inform recommendations for a provincial diversity strategy for 2014 and beyond, within the context of the broader HSF’s national strategic planning and priorities.

This RFP is to select a supplier to design and implement a needs assessment study of the service gaps and needs of Chinese Ontarians living with CCVD to inform recommendations for a provincial diversity strategy for 2014 and beyond, within the context of the broader HSF’s national strategic planning and priorities.

Key questions to be addressed by the needs assessment study are as follows:

  1. What gaps, needs or barriers to providing support to Chinese survivors of heart disease and stroke do other relevant stakeholders (e.g. families, communities etc) face?
  2. What gaps, needs or barriers do Chinese survivors of heart disease and stroke face related to self-management?
  3. What gaps, needs or barriers to optimal management do health care providers who serve Chinese survivors of heart disease and stroke face?
  4. What promising practices have other organizations implemented to help Chinese survivors of heart disease and stroke self-manage their chronic disease(s)?
  5. What key opportunities could be enacted by HSF to better support:
  • families and other stakeholders who support Chinese survivors of heart disease and stroke?
  • optimal management by health care providers who serve Chinese survivors of heart disease and stroke?

This needs assessment study may include the following components: a targeted literature review, key stakeholder interviews, a survey and/or focus groups.  The deliverable is a final report (in pdf format) by August 27, 2012 comprising detailed findings and recommendations to inform a provincial diversity strategy for 2014 and beyond.

This work must be completed for a total fixed price that does not exceed $20,000 including GST/HST.
