Reports to: Manager, Community Mission Team

Status: Full time; Permanent

Grade: C2

Purpose:

The Heart and Stroke Foundation’s Community Mission Team is dedicated to delivering programs and engaging with diverse communities by building partnerships and reaching out to key decision makers.  The incumbent plays an integral role in helping individuals in the North Eastern Ontario (Sudbury) Region reduce their risk and live longer, healthier lives.  The incumbent, supported by leadership volunteers, will engage communities for impact on the Foundation’s mission.  The incumbent will develop and implement an annual plan while responding to regional and local needs and priorities.

Please note:  this position travels frequently within the North Eastern Ontario region and therefore access to a vehicle is a requirement.

Key Accountabilities:

Strategies

  • Works with key stakeholders to pro-actively develop strategies to work on the mission around health promotion and advocacy in the community
  • Identifies and helps to remove barriers to long-term sustainable programs and policies to encourage and engage the community (programs may include reducing obesity in children and introduction of automated external defibrillators in public places)
  • Assesses where the community is in the public policy process by conducting needs assessments and helps to position the HSF appropriately in the public policy process
  • Positions the HSF to look at how the system and society may not be meeting the needs of the community and pro-actively identifies new programs and policies to address these gaps
  • Works with key stakeholders (school boards, community leaders, politicians, municipalities, health care professionals, recreation facilities, etc.) to bring about policy changes
  • Develops, implements and monitors an annual community engagement plan that reflects the province-wide mission priorities
  • Provides input on the development of a community based strategy to inform provincial goals and plans and completes an annual evaluation of the progress made against plan

Project Management

  • Project manages health promotion initiatives which includes stakeholders such as politicians, the community and health partners
  • Manages a significant portion of African-Canadian media coverage for the HSF within the community and coordinates same

Partnership

  • Builds, fosters and maintains partnerships within the community and is present at various group meetings, representing HSF “at the table” to forward its mission
  • Partners with the community and key stakeholders to further HSF’s mission by pro-actively identifying and helping to reduce the barriers preventing them from incorporating healthy living into their lives and thus developing long-term sustainable programs (i.e. childhood obesity)
  • Understands program deliverables and acts as ‘key gatekeepers’ with the key stakeholders from the community
  • Builds strong relationships with relevant leaders and key stakeholders within these communities and is sensitive to the need for customized messaging for diverse groups whilst building and maintaining trust partnerships
  • Acts as a resource and support to both the Community Mission Team, the Foundation and key community stakeholders
  • Partners with other HSF programs and work within the fundraising and communication environment to provide information to HSF fundraisers
  • Partners with First Nations and Aboriginal communities and key HSF stakeholders to ensure knowledge transfer

Leadership

  • Responsible for the strategic direction of the volunteer team including recruitment and management of volunteers from positions of leadership to ‘day of’ volunteers
  • Provides leadership and influences health partners and volunteers concerning provincial advocacy efforts

Key Challenges/Constraints:

  • Balancing the needs of a large geographical territory.
  • Maximizing opportunities and minimizing overlaps/confusion between existing fundraising programs and new/existing mission initiatives
  • Balancing the need to be responsive to communities as well as to province wide Foundation goals
  • Managing a large number of relationships and expectations with both internal and external stakeholders
  • Measuring the success of regional and local programs
  • Managing through situations where autonomous decisions have been made and implemented by self or others but within a set internal structure

Other Requirements:

  • 4+ years of health promotion, recreation, community development, public policy and advocacy, and/or government relations
  • Related post secondary education (i.e., public health, health promotion, public policy, adult education, community development)
  • Proven knowledge or experience with advocacy and/or policy development
  • Experience working with First Nation, Aboriginal communities
  • Extensive facilitation experience with multiple stakeholders and interest groups
  • Proven knowledge or experience recruiting and working with volunteers
  • Proven ability to persuade and lead
  • Proven ability to manage workload with little structure
  • Ability to speak a second language an asset
