Flemingdon Health Centre’s successful South Asian Diabetes Prevention Program (SADPP) is a unique and innovative, evidence-based program that is responsive to the needs of the South Asian communities. Started in 2009, the overall goal of this program is to provide these communities with equitable access to language-specific and culturally-relevant diabetes prevention and management services.

The program involves: conducting mass community outreach through places of worship and community centers, providing information sessions, mass screening for pre-diabetes using evidence-based tools and guidelines, referring those who are at high risk to appropriate services in the community and provision of supportive workshops for the caregivers. In addition, participants are enrolled in a follow-up workshop; Diabetes Prevention Workshop that builds skills and enhances knowledge around healthy eating, physical activity and mental health. The Program is funded by the Toronto Central LHIN.

We are seeking to hire a Community Outreach Worker a dynamic role as part of the program team.

The Community Outreach Worker will work in conjunction with a team consisting of a Community Health Nurse, a Dietician and a Program Coordinator, with the objective of outreaching and providing services pertaining to diabetes prevention and management to the South Asian community.  (Pending LHIN Funding)

As a Community Outreach Worker you will work with the SADPP Outreach Team to establish lasting connections with individuals and groups in a particular buildings or neighbourhoods for specific South Asian communities. The outreach activities will include engaging South Asian communities and establishing connection with seniors where they gather, like community centres or places of worship.  You may also have to organize meetings in places where people are likely to feel safe and comfortable to attend.

Position Type: Contract, July 2012 to March 2013, Part time: 21 hours per week; including evenings and weekends (Pending LHIN Funding)

Qualifications

  • Bachelor’s Degree or equivalent and minimum 3 years experience in a community engagement
  • Proficiency in two or more of the South Asian languages (any two is a major requirement);
  • First priority:, Gujarati, Bengali, Marathi, or Punjabi
  • Second Priority: Sindhi, Kannada, Malayalam, Oriya, Assamese, Hindi, Nepali
  • Demonstrated expertise with community outreach and engagement skills
  • Knowledge and sensitivity to the challenges facing the South Asian community
  • Knowledge of the organizations serving the South Asian community is a significant asset
  • Team player with demonstrated interpersonal and communication skills with high ability to exercise good judgment and tact
  • Exceptionally strong presentation, facilitation, organizational, and communication skills
  • Accurate word-processing, with knowledge of MS Word, Excel and PowerPoint
  • Able to work with people from diverse communities
  • Ability to relate well to volunteers and staff
  • Ability to assess priorities and meet deadlines
  • Proven capacity for initiative taking and ability to carry programs to completion with minimum supervision


Key Duties and Responsibilities

  • Conduct effective outreach to the South Asian community through networking and building effective relationships with the various faith based organizations, community centres and senior homes
  • Play key role in highlighting the community needs and challenges
  • Promote, develop and support the program activities, events and community fairs
  • Recruit and register groups of South Asian participants
  • Purchasing of program supplies as needed
  • Assist in the recruitment, training, supervision and engagement of volunteers
  • Consult with the diverse South Asian community and assist them to address  diabetes needs
  • Meet the program performance targets
  • Gather and organize data, maintain accurate records, submit weekly progress reports
  • Participate in community events and committees as required
  • Gather and maintain diabetes program information as required
  • Work collaboratively with the program team to ensure activities and approaches are culturally relevant to the South Asian community and meet their needs
  • Answer phone calls and correspondence pertinent to program responsibilities
  • Any other duties as directed by Program Coordinator
