The Ministry of Health and Long-Term Care (MOHLTC) approved that the North Simcoe Muskoka LHIN will take on the function of the North Simcoe Muskoka Diabetes Regional Coordination Centres (DRCCs) effective February 1, 2013.  The functions of the DRCC Program and the oversight of the Diabetes Education Programs (DEPs), aligns with the NSM LHIN’s Care Connections vision and, in particular, our efforts surrounding chronic disease prevention and management.

Primary Purpose

Responsible for the development and implementation of short and long-term strategies to enhance delivery of chronic disease programs and services with initial focus on diabetes, vascular and lung health; and conduct outreach initiatives in the area of Chronic Disease Prevention and Management (CDPM).

Key Responsiblities

  • Builds awareness of diabetes and vascular health services and participates in the dissemination of diabetes standards of practice among providers across the LHIN
  • Facilitates Quality Improvement initiatives across the spectrum of diabetes services (Diabetes Education Programs, Community Health Centres (CHCs), Family Health Teams (FHTs), Primary Care Practices) and local Health Links
  • Works collaboratively with other groups and organizations in the region (primary care, physicians, FHT, nurse practitioners, LHINs, diabetes education programs, CHC and other agencies) to assist with the coordination of chronic disease prevention and management program and service delivery
  • Participates and contributes to provincial and inter-LHIN planning activities
  • Collaborates with other LHIN staff on work plans and priorities
  • Acts as a resource to identify and provide advice on engagement objectives and the selection and use of appropriate methodologies through the development of implementations of community engagements
  • Participates in stakeholder consultations and provides analysis of information and data necessary to foster effective discussion
  • Undertakes an outreach program to the LHIN area, developing and maintaining formal and informal networks to ensure their knowledge of available programs and services
  • Participates in the planning and development of LHIN’s Diabetes Strategy programs/services by providing information and reports regarding community outreach activities as needed
  • Participates in the conduct of evaluations of effectiveness of current programs
  • Manages preparation of reports as appropriate for the CEO, Executive Management and Board of Directors
  • Participates in special projects as they arise

Position Requirements

Minimum Education:

  • Post-secondary education in health administration, finance, health planning, business administration, research or equivalent experience in a health care environment with  a minimum of five (5) years of experience

Preferred Experience:

  • Significant experience with outreach initiatives and program development
  • Demonstrated skills and experience in Health Care Quality Improvement Strategies including Quality Coaching
  • In-depth knowledge of primary care issues, health care system stakeholders and challenges with Diabetes and chronic disease management
  • Demonstrated leadership, planning and communication skills
  • Solid interpersonal, organizational and critical-thinking competencies
  • Excellent written communication skills to prepare clear reports, which will support effective decision making and priority setting
  • Proficiency in community development activities, group facilitation and outreach processes
  • Ability to resolve conflicts
  • Strong commitment to diversity, equity, partnerships and advocacy with regards to service delivery and program planning
  • Strong teamwork and customer service skills
