The vision of the South West Regional Wound Care Program is: “The South West health system partners will become leaders in integrated Wound Care management.” Objectives to achieve this vision include:

  • Ongoing development and sustainability of an evidence-based wound care framework based on RNAO and Canadian Association of Wound Care leading practices;
  • Ongoing development and maintenance of a sustainable knowledge translation framework to support all care providers across all care settings;
  • Ongoing alignment of wound product selection throughout the South West region;
  • Continuous Quality Improvement through measurement and evaluation of wound care outcomes in collaboration with all health system partners in the South West region.

The South West Regional Wound Care Program is hosted by the South West CCAC but supports all hospitals, LTC homes and community providers. As a member of the Performance Management and Accountability team, the Program Lead for the Regional Wound Care Program ensures integrated delivery of wound care that spans the continuum of care. The Program Lead drives improvement in the outcomes for people with wounds in the South West Region. In collaboration with health system partners, the Program Lead formulates and executes a coordinated strategy for the delivery of wound care based on leading practice. The Program Lead for the Regional Wound Care Program leads and participates in and demonstrates an understanding of quality, risk and client safety principles and practices. The Program Lead follows all safe practice and procedures to support a safe client and working environment.

Reporting to the Senior Director, Performance Management and Accountability, qualifications and experience will include:

  • Post-secondary education in health science/administration, health promotion or related field or an equivalent combination of education and experience
  • 3-5 years of leadership/management experience with the ability to manage complex projects involving multiple agencies, budgets and stakeholders
  • A proven track record of effective working relationships with internal and external customers and clients and the ability to build teams and lead through system level change
  • An ability to resolve conflicts and achieve outcomes
  • Excellent time management skills and ability to prioritize multiple, conflicting demands
  • Superior organizational skills and solid knowledge of project management and quality improvement methodology
  • Experience and working knowledge related to service delivery systems across the continuum of care
  • Knowledge of wound care throughout the South West LHIN area an asset
  • Ability to use MS Office applications (e.g., Word, Excel, PowerPoint, etc.)
  • Excellent interpersonal, diplomacy, communication and presentation skills
  • Strong collaboration skills to participate on projects and committees with colleagues across the South West CCAC or wound care providers
  • Ability to deliver information effectively and interpret data clearly in a variety of settings including one-on-one with staff and in team meetings
  • Ability to deal constructively with a variety of contentious/difficult situations, discuss sensitive information and influence/persuade others to follow a recommended course of action
  • Good coaching and communication skills to provide instructions and guidance to staff with respect to activities, challenges and questions

The successful candidate for this part time position will be employed by the South West CCAC. The days of work are negotiable as is the work site but proximity to London is required. Extensive travel throughout the South West region and periodic travel outside of the South West region is required.
