The North Simcoe Muskoka Community Care Access Centres (CCAC) is one of fourteen across the province. These publicly funded healthcare agencies are dedicated to enhancing the health, quality of life and independence of individuals in our communities by offering a single point of access to home and community services. The North Simcoe Muskoka CCAC is home to close to 453,710 people and encompasses the District of Muskoka, most of the County of Simcoe and a portion of Grey County.

As part of a system side initiative to strengthen connections across the health continuum, the North Simcoe Muskoka CCAC is hiring 4 Nurse Practitioners to provide enhanced support, navigation and leadership to complex palliative clients within North Simcoe Muskoka catchment area.

Nurse Practitioner

As an integral member of this palliative care team, you will provide holistic patient care to complex palliative clients and provide care connections across health care sectors for our clients in the community. Working collaboratively across the health care system, the Palliative Care Nurse Practitioner will provide expert clinical palliative leadership to support seamless, integrated care delivery.

As a key member of the Client Services Team, you will be involved in all domains of advanced practice nursing, including mentoring and coaching case managers, service providers, nursing and physician colleagues, and participating in educational initiatives to advance evidence-based practice in palliative care.

Qualifications: current registration with the College of Nurses of Ontario in the Extended Class and graduate of a Nurse Practitioner Program with BScN. Successful candidates will have experience in Palliative Care Nursing, preferably in a community setting. You will also have demonstrated advanced knowledge in consultation and ethical decision-making and ability to apply theory and evidence to advance clinical practice and outcomes. To be successful in this position, you will also have excellent interpersonal and communications skills to communicate clients and family members. You must also have valid driver's license and access to a vehicle.
