The Health and Social Services Department provides services to Norfolk County and Haldimand County which are single tier municipalities located on the North Shore of Lake Erie in Southwestern Ontario.   Both Counties offer abundant recreational and lifestyle opportunities.

Public Health Nurse (TFT)

The Infectious Disease Team’s focus is to ensure the Infectious Disease prevention and control standard and the TB standard are achieved. As a member of this team your duties may include however are not limited to Community ID/STI surveillance, Infection Control and Outbreak management.

  • Contributes to health promotion and disease prevention initiatives to individuals and the community.
  • Provides nursing service for individuals, families, groups and the community according to the program specific policies and procedures, the program’s operational requirements.
  • Meets the professional practice standards as defined by the College of Nurses of Ontario.
  • PHN’s will meet and adhere to the Current Canadian Community Health Nursing Standards of Practice, and the Core Competencies as defined by the Public Health Agency of Canada and Public Health Nursing Discipline Specific Competencies.

The ideal candidate will have a Baccalaureate Degree in Nursing and minimum two years experience in nursing preferred. Must have: A current Certificate of Competence with the Ontario College of Nurses. Current certification in CPR. Proven organization skills. Proven ability to work both independently and in a team environment. Proven creative (program development) and evaluation (of program) skills. A valid Ontario driver’s license and have access to a reliable vehicle as travel is required. Proficient oral and written communication skills including computer literacy and proficiency in corporate standard software.
