Primary Care Nurse Practitioner - The Youth Centre
1 Temporary Full Time Position

Position Description

As a member of the interdisciplinary team, the Nurse Practitioner (NP) will provide primary health care services to youth 13 to 29 years of age with a strong emphasis on health promotion, and illness prevention operating within the values and principles and strategic direction of the Youth Centre. Reporting to the Clinical Manager the successful candidate will have the opportunity to provide care on a variety of issues, including general primary, sexual, and reproductive health care.


  • Provide direct, comprehensive clinical care within the full scope of RN (EC) role
  • Excellent health assessment skills combined with sound clinical judgement, diagnostic, and treatment¬† skills
  • Demonstrated ability to assess, plan, implement, and evaluate health promotion programs based on identified needs of client and community
  • Participate in quality improvement initiatives maintaining accurate, complete, and timely medical records, and participating in chart reviews
  • Demonstrate leadership by being able to work effectively in an interdisciplinary team environment with excellent communication and interpersonal skills with both medical and non-medical staff or independently
  • Advocate, educate, lead, provide mentorship, and consultation to both medical, non-medical staff and clients as required
  • Integrate research and best practice guidelines into practice
  • Commitment to and knowledge of community-based health care and the ability to identify and link clients to services and resources


  • Baccalaureate or Masters degree in nursing preferred
  • Completion of a recognized Primary Care NP Program NP-PHC
  • Current member in good standing with the College of Nurses of Ontario as RN (Extended Class)
  • Membership in a professional organization is an asset (i.e. NPAO, RNAO)
  • Minimum of two years experience working with youth in a primary health care setting, paediatric experience is an asset
  • Experience with public speaking, and presentations to internal and external stakeholders in the community
  • Basic CPR
  • Demonstrated critical thinking and problem solving skills
  • Awareness and sensitivity to the social determinants of health with a non-judgmental approach to service provision
  • Excellent organizational, flexible, and time management skills
  • Able to work flexible hours (evening work required)
  • Experience with electronic medical records (Nightingale, Purkinjie, and Microsoft Office Suite)
  • Subject to a Police Reference Check
  • Current and valid Ontario's driver's license and access to a vehicle
