This free conference will bring together health system leaders and providers to share lessons learned on integrating care for patients with complex health and social needs. Best known for the development of the Chronic Care Model, Dr. Ed Wagner, Director (Emeritus), MccColl Center for Health Care Innovation at the Group Health Research Institute will be the keynote speaker for the conference. He will share insights from his work on care integration and draw on his experiences to provide feedback on the work being done by BRIDGES (http://www.building-bridges.ca/).

Building Bridges to Integrate Care (BRIDGES) is an incubator for innovative models to integrate care across the continuum and across disciplines for people with complex chronic disease. Within BRIDGES, academics and health service providers work closely to perform quality improvement activities and conduct outcome evaluation using mixed methods. BRIDGES:
- Is designed to integrate across silos.
- Works at the interface of hospital, community and primary care.
- Works with institutions and front-line providers incorporating methodologies that include quality improvement, implementation evaluation and mixed methods evaluation.
