We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and east downtown area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, housing assistance, harm reduction and homeless programs, Pathways to Education for high school students, infant and child development programs, parenting education and family advocacy.  We currently have the following opening on our Community Health team

Registered Dietitian – Community Diabetes Educator
Permanent, 28 hours per week
Salary Range:  $43,767 to $52,731 plus benefits

You will work in partnership with a Community Diabetes Educator-Registered Nurse (CD-E) and within a multi-disciplinary team to provide primary care to clients with diabetes.  You will also provide individual diabetes education counselling, community outreach and health promotion/education to families and the community.

Responsibilities:

  • provide thorough holistic assessment, monitoring and education for clients with diabetes taking into consideration the client’s level of knowledge, readiness, socio-economic and cultural background
  • develop client centered care plans related to diabetes
  • ensure continuity of care by maintaining up-to-date case notes, communication with the client's primary care provider and other service providers as required
  • monitor clients regularly, participate in chart reviews and case management
  • engage in health promotion activities to groups and individuals on and off site in conjunction with other service providers
  • provide outreach to/liaise with local community, organizations, networks and health care providers
  • in collaboration with Diabetic Nurse, play a leadership role in the development of the Centre’s diabetic education program, including program promotion, development of materials and evaluation
  • consult and collaborate with other multidisciplinary staff as appropriate in program development, service delivery and evaluation
  • maintain statistical data for funding purposes  
  • maintain and develop professional competence

Qualifications:

  • university degree in food and nutrition from a recognized university
  • completion of an accredited Dietetic Internship
  • work experience as a Registered Dietitian
  • registered member of the College of Dietitians of Ontario
  • Certified Diabetes Educator (CDE)
  • proven clinical skills including health promotion teaching ability; group facilitation experience
  • two years’ current practice within a Diabetes Education Program or equivalent
  • ability to take the initiative and work independently
  • ability to work flexible hours
  • strong written and oral communication skills, and strong computer skills
  • fluency in additional languages, especially Tamil, Bengali and Somali
  • Ability to work collaboratively within and across teams
  • Understanding of Anti-oppression principles and ability to work effectively with people of various cultural, age and economic backgrounds
  • Knowledge of the Regent Park area and issues affecting the community
